Researchers at the IRCCS Materno Infantile Burlo Garofolo have identified a unique population of extracellular vesicles (EVs) in the follicular fluid of women undergoing fertility treatments. Follicular fluid surrounds developing eggs in the ovaries and creates a highly specialized environment where the immune system must be carefully regulated to support healthy egg maturation.
In many biological systems, small extracellular vesicles (sEVs) that carry a protein called PD-L1 are known to suppress immune activity, preventing immune cells from attacking healthy tissues. The research team wanted to know whether similar immune-regulating vesicles exist in human follicular fluid, where maintaining immune balance is essential for successful reproduction.
Using highly sensitive imaging and molecular labeling techniques, including single-particle interferometric reflectance imaging sensing and atomic force microscopy, the team analyzed EVs from the follicular fluid of ten women. They focused on three common vesicle surface proteins: CD63, CD81, and CD9, along with PD-L1 to see how these markers were distributed and whether PD-L1 was linked to a particular type of EV.
The results showed that most tetraspanin-positive vesicles in follicular fluid were smaller than 50 nanometers, and that PD-L1 was most often associated with CD9-positive vesicles. This consistent pattern across patients indicates that CD9+ PD-L1-bearing EVs may represent a distinct and stable subpopulation with potential immunoregulatory functions in the ovary.
These findings suggest that PD-L1+ extracellular vesicles may help promote immune tolerance in the follicular environment, ensuring that egg development proceeds without harmful immune interference. Moreover, these vesicles could serve as biomarkers for evaluating immune balance and fertility potential in reproductive medicine. Understanding their role could eventually lead to better diagnostics and therapeutic strategies for improving fertility outcomes.
